The Medi, Inc.

The Medi Inc, is a 501c3 nonprofit organization whose mission is to connect community members to essential health and social care resources in their area and the organizations that provide them, aiming to reduce health disparities and promote healthy equity while addressing Social Determinants of Health (SDoH). The Medi is dedicated to cultivating a culture of collaboration, working alongside Community-Based Organizations (CBOs), faith-based organizations, social service providers, hospitals, insurance carriers, and any organizations that focus on people-centered care and connection. Through our hallmark project, The Medi Community Resource Center, our vision is to create an ecosystem of care that builds a health and social care safety net, strengthening communities while pursuing the goal of developing more equitable communities of care, improving health outcomes, and multiplying the impact of healthy community members and the organizations that support communities.
